Physical Therapy - A physician's referral is necessary for evaluation and treatment.
Appropriate referrals are children whose problems are primarily physical in nature, such as spina bifida, cerebral palsy, juvenile rheumatoid arthritis, muscular dystrophy, trauma, high risk infants and other orthopedic and neurological conditions.
Occupational Therapy - A physician's referral is necessary for evaluation and treatment.
Referrals considered for treatment include the child with sensory motor and learning disabilities as well as the physical conditions listed under Physical Therapy above.
Speech-Language Therapy - Referrals are accepted from any source, but contact is maintained with the child's physician.
Appropriate referrals include children with speech-language delays which may or may not be associated with oral-facial anomalies, neurological disorders, hearing loss, voice, resonance and rhythm, as well as certain chromosomal alterations or syndromes.
Preschool Vision Screening - Children from ages 2 1/2 to six years of age are screened in preschools or daycare centers and "open" screenings, with the objective of identifying children with amblyopia ("lazy eye") or potential amblyopia. Referral is made to an eye specialist of the parent's choice.
For therapy, those children:
- who are under 18 years of age.
- who are not eligible for other therapy services because of geographical location, transportation difficulties, diagnoses, insurance or other limiting conditions.
- whose parents are willing to be actively involved in the therapy program.
- who usually can be seen in a home setting.
- who are not enrolled in or eligible for a program providing the same service. The Major Project is not allowed to duplicate or supplement services.
